В зависимости от вида проведенного оперативного лечения пациенты были разделены на две группы: 1-я группа — 28 пациентов (31 глаз), которым провели кросслинкинг роговицы с персонализированной эксимерлазерной фотоабляцией по предложенной нами методике; 2-я группа — 27 пациентов (33 глаза), которым выполняли кросслинкинг роговицы по стандартной методике (Дрезденский протокол).</p></sec><sec><title>Результаты</title><p>Результаты. Значение некорригированной остроты зрения достоверно не отличалось между группами на протяжении всего периода наблюдения, а максимально корригированная острота зрения была достоверно выше с 1-го мес. в 1-й группе. Начиная с 6-го мес. после лечения значение центральной толщины роговицы было статистически больше в 1-й группе, а значение минимальной толщины не имело достоверных различий на всем протяжении наблюдения в течение 1 года.</p><p>Максимальная кератометрия стала достоверно меньше уже с 1-го мес. после хирургического лечения в 1-й группе и сохранялась такой на протяжении всего периода наблюдения. Послеоперационных осложнений не было ни в одной группе.</p></sec><sec><title>Заключение</title><p>Заключение. Предложенный метод кросслинкинга роговицы с персонализированной эксимерлазерной фото- абляцией обеспечивает безопасное и эффективное лечение кератоконуса и обладает стабилизирующим эффектом на прогрессирующий характер заболевания. В то же время за счет применения эксимерного лазера снижается длительность роговичного синдрома, уменьшается иррегулярность роговицы и улучшаются зрительные функции в послеоперационном периоде, что приводит к скорейшей зрительной, профессиональной и социальной реабилитации пациентов с кератоконусом после выполнения кросслинкинга роговицы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To develop a modiﬁed corneal crosslinking technique with personalized excimer laser photoablation and evaluate its eﬀectiveness and safety.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 55 patients (64 eyes) with stage 1—3 keratoconus (Amsler — Krumeich classiﬁcation) with conﬁrmed progressive course. Depending on the type of surgical treatment performed, the patients were divided into 2 groups: Group 1 — 28 patients (31 eyes) who underwent corneal crosslinking with personalized excimer laser photoablation, according to our proposed technique, and Group 2 — 27 patients (33 eyes) who underwent corneal crosslinking according to the standard technique (the «Dresden Protocol»).</p></sec><sec><title>Results</title><p>Results. The value of uncorrected visual acuity did not diﬀer signiﬁcantly between the groups throughout the entire observation period, and the best-corrected visual acuity was signiﬁcantly higher from the ﬁrst month in Group 1. The central corneal thickness was statistically greater in Group 1 starting from the 6th month after treatment, and the value of the minimum thickness did not diﬀer signiﬁcantly throughout the observation period. Maximum keratometry became signiﬁcantly lower already from the 1st month after surgical treatment in Group 1, and remained so throughout the observation period. There were no postoperative complications in any group.</p></sec><sec><title>Conclusions</title><p>Conclusions. The proposed combined method of corneal crosslinking with excimer laser photoablation provides safe and eﬀective treatment of keratoconus and has a stabilizing eﬀect on the progressive nature of the disease.
