Подагра нередко сочетается с сердечно-сосудистыми заболеваниями и является независимым фактором риска развития кардиоваскулярных событий. Лечение направлено на купирование обострений и прием лекарственных средств для снижения уровня мочевой кислоты до целевых значений, что обеспечивает отсутствие приступов. В начале уратснижающей терапии у многих пациентов могут возникать обострения артрита. С целью их предотвращения проводится противовоспалительная профилактика. Основные препараты для ее проведения — колхицин, нестероидные противовоспалительные средства, глюкокортикостероиды. Оптимальный препарат для противовоспалительной профилактики обострений подагры у лиц с заболеваниями системы кровообращения должен соответствовать нескольким требованиям: эффективно предотвращать обострения, не иметь противопоказаний к назначению при сердечно-сосудистых заболеваниях и в идеале снижать сердечно-сосудистые риски. Цель исследования — проанализировать наиболее часто применяемые в практике препараты на соответствие этим требованиям.</p></abstract><trans-abstract xml:lang="en"><p>Gout is a chronic disease caused by the deposition of monosodium urate crystals in tissues, which most commonly manifests as inflammatory arthritis with a marked pain syndrome. Gout is often combined with cardiovascular disease and is an independent risk factor for cardiovascular events. Treatment includes managing exacerbations and taking medications to reduce uric acid levels to target values that ensure the absence of flares. At the beginning of urate-lowering therapy, many patients may experience exacerbations of arthritis. In order to prevent them, anti-inflammatory prophylaxis is carried out. The main drugs for anti-inflammatory prophylaxis are colchicine, nonsteroidal anti-inflammatory drugs, glucocorticoids. The optimal drug for anti-inflammatory prophylaxis of gout exacerbations in persons with cardiovascular disease should meet several requirements: effectively prevent gout exacerbations, have no contraindications for prescription in cardiovascular disease, and, ideally, reduce cardiovascular risks.
