Изучить структуру расстройств мочеиспускания у пациентов с повреждениями спинного мозга на различных уровнях.Материал и методы. Обследованы 60 пациентов с повреждениями спинного мозга и наличием нервно-мышечной дисфункции мочевого пузыря. Диагноз выставлялся на основании представленного алгоритма диагностики. Отведение мочи пациентами осуществлялось четырьмя основными путями: 1) неуправляемое мочеиспускание или ассистируемое опорожнение мочевого пузыря («выстукивание», «выдавливание») и/или использование уропрезерватива, памперса - у 20 (33,4 %) пациентов; 2) ассистируемое опорожнение мочевого пузыря и самокатетеризация - у 20 (33,4 %) пациентов; 3) периодическая самокатетеризация - у 18 (29,9 %) пациентов; 4) постоянный уретральный катетер/цистостома были у 2 (3,3 %) пациентов.Результаты. Анализ 60 пациентов с нервно-мышечной дисфункцией мочевого пузыря после травматического повреждения спинного мозга показал обоснованным выделить 2 основных типа расстройства мочеиспускания: 1) пациенты с сохраненным самостоятельным мочеиспусканием, а также пациенты с частично сохраненным самостоятельным мочеиспусканием и самокатетеризацией (1-я группа, 33,3 %); 2) пациенты, у которых отсутствует самостоятельный акт мочеиспускания, отведение мочи осуществляется только самокатетеризацией, постоянным уретральным катетером или с помощью цистостомы (2-я группа). Пациенты 1-й группы - это пациенты с гиперрефлексией детрузора (с гиперрефлекторным мочевым пузырем), пациенты 2-й группы - с гипорефлексией детрузора (с гипорефлекторным мочевым пузырем).Заключение. Предложенное разделение на группы оправдано с точки зрения различия в отведении мочи, а также имеющимися различиями в причинах и уровнях повреждения спинного мозга, емкости мочевого пузыря и осложнений со стороны верхних мочевых путей, и может быть использовано при планировании лечения.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To study the structure of neurogenic urination disorders in patients with spinal cord injuries at various levels.Materials and methods. 60 patients with spinal cord injuries and neuromuscular bladder dysfunction were examined. The diagnosis was made on the basis of the presented diagnostic algorithm. Urine diversion by patients was carried out in four main ways: 1) uncontrolled urination or assisted emptying of the bladder («tapping», «squeezing») and / or using a urocondom, diaper -- in 20 patients (33,4 %); 2) assisted emptying of the bladder and self-catheterization - in 20 patients (33,4 %); 3) periodic self- catheterization -- in 18 patients (29,9 %); 4) an indwelling urethral catheter/cystostomy was present in 2 (3,3 %) patients.Results. Thus, an analysis of 60 patients with neuromuscular dysfunction of the bladder after traumatic spinal cord injury showed that it is reasonable to distinguish 2 main types of urination disorders: 1) patients with preserved spontaneous urination, as well as patients with partially preserved independent urination and self-catheterization (1 group,33,3 %). 2) patients who do not have an independent act of urination, urine diversion is carried out only by self-catheterization, a permanent urethral catheter or using a cystostomy (2 group). Patients of the first group are patients with detrusor hyperrefflexia (with a hyperreflex bladder), patients of the second group - with detrusor hyporeflexia (with a hyporeflex bladder).Conclusion.
