Проанализировать факторы, оказывающие влияние на отдаленные результаты реконструктивно-восстановительных операций на артериях нижних конечностей.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Выполнен ретроспективный анализ 98 медицинских карт пациентов с хроническими облитерирующими заболеваниями артерий нижних конечностей после открытых операций на артериях бедренно-подколенной области. Сформированы две группы пациентов: 1-я группа (n = 34) – с перемежающейся хромотой; 2-я группа (n = 64) – с критической ишемией. Обработку результатов исследования проводили методами прикладной статистики с использованием программы MS Excel и программного пакета для статистического анализа Statistica 8.0. Статистически значимыми считали различия при р &lt; 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. По данным МСКТ-ангиографии, III стадию по классификации GLASS имели 58,82 % пациентов с хронической артериальной недостаточностью (ХАН) 2б степени, 71,31 % пациентов с ХАН 3 и 4 степени. При этом протяженность окклюзии поверхностной бедренной артерии у пациентов с ХАН 2б степени составила 230 [200; 300] мм, у пациентов с ХАН 3, 4 степени – 300 [105; 300] мм (p &gt; 0,05). Положительный результат после оперативного вмешательства наблюдали в 79,41 % случаев у пациентов с перемежающейся хромотой и в 46,88 % случаев у пациентов с хронической ишемией, угрожающей потерей конечности (p &lt; 0,05). Сохранность конечности в течение 2 лет после реваскуляризации у пациентов 1-й группы составила 88,24 %, у пациентов 2-й группы – 70,31 %. У пациентов, принимавших антиагрегантные препараты и соблюдавших отказ от курения, чаще отмечали положительный эффект после операции (p &lt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Пациенты с атеросклеротическим поражением артерий нижних конечностей после открытых реконструктивных операций имели низкую приверженность к консервативной терапии и к изменению образа жизни (все рекомендации соблюдали в 8,2 % случаев). Основные неблагоприятные события на конечностях у пациентов, не принимавших антиагреганты и не соблюдавших отказ от курения, возникали достоверно раньше, чем у пациентов, соблюдавших рекомендации в отдаленном периоде (p &lt; 0,05).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To analyze the factors influencing the long-term results of reconstructive surgeries on the arteries of the lower extremities.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Retrospective analysis of 98 medical records of patients with chronic obliterating diseases of the arteries of the lower extremities after open surgeries on the arteries of the femoral-popliteal region. Two groups of patients: the first group – with intermittent claudication (n = 34), the second group – with critical ischemia (n = 64). The results of the study were processed by applied statistics methods using MS Excel and the software package for statistical analysis Statistica v. 8.0. Differences were considered statistically significant at p &lt; 0.05.</p></sec><sec><title>Results</title><p>Results. According to angiography, stage III according to the GLASS classification was present in 58.82 % of patients with CLI 2b, in 71.31 % of patients with CLI 3, 4. The length of the SFA occlusion in patients with CLI 2b is 230 [200; 300] mm, with CLI 3, 4 – 300 [105; 300] mm. A positive result after surgery was observed in 79.41 % of cases in patients with intermittent claudication and in 46.88 % of cases in patients with chronic ischemia threatening limb loss (p &lt; 0.05). Limb preservation within 2 years after revascularization is 88.24 % in patients of the first group, 70.31 % in patients of the second group. Patients taking antiplatelet drugs and abstaining from smoking were more likely to have a positive effect after surgery (p &lt; 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. Patients with atherosclerotic lesions of lower extremity arteries after open reconstructive surgeries have low adherence to conservative therapy and lifestyle changes (all recommendations were followed in 8.2 % of cases).
