Провести анализ ограничений жизнедеятельности и связанных с ними нарушений функций организма у пациентов с туберкулезом органов дыхания (ТБ ОД), разработать и валидизировать объективные правила прогнозирования экспертных оценок степени выраженности ограничения категорий жизнедеятельности при проведении медико-социальной экспертизы (МСЭ) и установлении инвалидности.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 149 пациентов с ТБ ОД, впервые направленных на МСЭ для установления инвалидности, и 75 пациентов, успешно завершивших курс лечения (контрольная группа). Исследование проводилось на базе УЗ «Минская городская медико-реабилитационная экспертная комиссия», УЗ «Минский областной противотуберкулезный диспансер», консультативно-поликлинического отделения ГУ «Республиканский научно-практический центр медицинской экспертизы и реабилитации» в период с 2017 по 2024 г. Средний возраст (SE) пациентов в исследуемой когорте составил 47,4 (0,71) года.</p></sec><sec><title>Результаты</title><p>Результаты. В результате анализа вариабельности ограничений категорий жизнедеятельности у пациентов с ТБ ОД установлено, что категория «способность к трудовой деятельности» является доминирующей в оценке функционирования и является ключевой при установлении инвалидности, однако категории «способность к самостоятельному передвижению» и «способность к самообслуживанию» также демонстрируют существенную вариабельность и значимость для экспертной оценки при установлении инвалидности. Для категорий «способность к самостоятельному передвижению» и «способность к самообслуживанию» при проведении экспертной оценки степени выраженности ограничений наиболее значимым предиктором выступила функция дыхания с точностью 92,6 % и 94,6 % на обучающей выборке и 90,7 % и 93,3 % – на тестовой соответственно. Для категории «способность к трудовой деятельности» основным предиктором стала функция иммунитета с точностью 96,6 % на обучающей выборке и 97,3 % – на тестовой, что подтверждает высокую устойчивость. Валидация на независимой тестовой выборке продемонстрировала незначительное снижение точности (разница ≤ 1,9 п. п., p = 0,61), что свидетельствует об отсутствии переобучения, высокой воспроизводимости и клинической пригодности разработанных правил. Результаты подтверждают их надежность и потенциал для практического применения в экспертной практике.</p></sec><sec><title>Заключение</title><p>Заключение. Для объективной экспертной оценки ограничений жизнедеятельности у пациентов с ТБ ОД в исследовании предложены простые алгоритмы, основанные на оценке степени выраженности нарушений функций дыхания и иммунитета. Их внедрение способствует стандартизации процесса МСЭ и повышению качества принимаемых экспертных решений, что спо собствует обоснованному определению группы инвалидности и оптимизирует разработку индивидуальных программ реаби литации и абилитации инвалида.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To conduct an analysis of limitations in life activities and associated impairments of bodily functions in patients with tuberculosis of the respiratory organs, to develop and validate objective rules for predicting expert assessments of the degree of severity of limitations in categories of life activities during medical and social assessments and the establishment of disability.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 149 patients with respiratory tuberculosis, who were referred for the first time for medical and social examination to determine disability, and 75 patients in the control group who successfully completed the course of treatment. The study was conducted at the Minsk City Medical and Rehabilitation Expert Commission, the Minsk Regional Тtuberculosis Dispensary, and the Consultative Polyclinic Department of the National Science and Practice Centre of Medical Assessment and Rehabilitation from 2017 to 2024. The mean age (SE) of the patients in the study cohort was 47.4 (0.71) years.</p></sec><sec><title>Results</title><p>Results. An analysis of the variability of activity limitations in patients with pulmonary tuberculosis revealed that the “work capacityˮ category is dominant in the assessment of functioning and is key in determining disability. However, the “independent mobilityˮ and “self-careˮ categories also demonstrate significant variability and are important for expert assessment when determining disability. For the “independent mobilityˮ and “self-careˮ categories, respiratory function emerged as the most significant predictor in the expert assessment of the severity of limitations, with an accuracy of 92.6 % and 94.6 % in the training set and 90.7 % and 93.3 % in the test set. For the “work capacityˮ category, immune function emerged as the primary predictor, with an accuracy of 96.6 % in the training set and 97.3 % in the test set, confirming their high stability. Validation on an independent test set demonstrated a slight decrease in accuracy (difference ≤ 1.9 percentage points, p = 0.61), indicating the absence of overfitting, high reproducibility, and clinical suitability of the developed rules. The results confirm their reliability and potential for practical application in expert practice.</p></sec><sec><title>Сonclusion</title><p>Сonclusion. The study proposes simple algorithms based on assessing the severity of respiratory and immune dysfunction for objective expert assessment of disability in patients with respiratory tuberculosis.
