Оценить отдаленные результаты лечения пациентов с применением хирургического эндоскопического метода и классических наружных доступов.Материал и методы. Были использованы и проанализированы данные двух групп пациентов, проходивших хирургическое лечение в РНПЦ ОМР им. Н. Н. Александрова. Первая группа (70 пациентов), ретроспективная, получала лечение с применением классических наружных доступов как в самостоятельном варианте, так и в рамках комбинированного или комплексного подхода в период с 2009 по 2018 г. Вторая группа (44 пациента), проспективная, проходила эндоскопическое эндоназальное хирургическое лечение с 2019 по 2022 г.Результаты. Анализ отдаленных результатов лечения пациентов, получавших эндоскопическое лечение (проспективная группа) и классические наружные доступы (ретроспективная группа), показал их сопоставимость. Общая однолетняя выживаемость составила 90,2 ± 4,6 % в проспективной группе и 95,7 ± 2,4 % — в ретроспективной, трехлетняя — 69,0 ± 8,2 % и 74,3 ± 5,2 % соответственно (р = 0,892). Скорректированная однолетняя выживаемость была 97,4 ± 2,6 % и 98,5 ± 1,5 %, трехлетняя — 76,8 ± 8,1 % и 80,6 ± 4,8 % соответственно (р = 0,939). Безрецидивная однолетняя выживаемость составила 90,2 ± 4,7 % в проспективной группе и 81,0 ± 4,7 % — в ретроспективной, трехлетняя — 73,6 ± 7,2 % и 62,8 ± 5,9 % соответственно (р = 0,608).Заключение. Учитывая сопоставимые онкологические показатели обеих групп, следует отметить, что применение разработанного метода эндоскопического эндоназального хирургического лечения пациентов со злокачественными опухолями околоносовых пазух и полости носа является целесообразным. Этот метод позволяет избежать обезображивающих разрезов на лице, повреждений верхнего альвеолярного нерва и западения мягких тканей щеки, которые возникают при удалении передней стенки верхнечелюстной пазухи. Использование видеоэндоскопического оборудования с оптикой различных углов позволяет выполнить более тщательное удаление опухоли под визуальным контролем в крыловидной и подвисочной ямках, проекции основной пазухи, ситовидной пластинки и других отделах основания черепа. Это значимо улучшает функциональный и эстетический результат хирургического лечения, не снижая его радикальности. </p></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To evaluate the long-term results of treatment of patients using surgical endoscopic method and classical</p></sec><sec><title>external accesses</title><p>external accesses.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Within the framework of the present study, the data of two groups of patients undergoing surgical treatment at the N. N. Aleksandrov NCC of Belarus were used and analysed. The first group, retrospective (70 patients), was treated using classical external accesses, both independently and as part of a combined or complex approach, in the period from 2009 to 2018. The second group, prospective (44 patients), underwent endoscopic endonasal surgical treatment from 2019 to 2022.</p></sec><sec><title>Results</title><p>Results. The analysis of the long-term results of patients treated with endoscopic treatment (prospective group) and classical external access (retrospective group) showed their comparability. Recurrence-free one- year survival was 90.2 ± 4.7 % in the prospective group and 81.0 ± 4.7 % in the retrospective group, three-year survival was 73.6 ± 7.2 % and 62.8 ± 5.9 %, respectively (p = 0.608).</p></sec><sec><title>Conclusion</title><p>Conclusion. Taking into account comparable oncological indicators of both groups, it should be noted that the application of the developed method of endoscopic endonasal surgical treatment of patients with malignant tumours of paranasal sinuses and nasal cavity is reasonable. This method allows to avoid disfiguring facial incisions, damage of the upper alveolar nerve and soft tissues depression of the cheek, which occur when removing the anterior wall of the maxillary sinus. The use of video endoscopic equipment with optics of different angles makes it possible to perform a more precise removal of the tumour under visual control in the pterygopalatina and infratemporalis fossae, the projection of the sinus sphenoidalis, the lamina cribrosa and other parts of the skull base.
