Установить факторы, ассоциированные с развитием метаболических нарушений у новорожденных от матерей с сахарным диабетом (СД) 1-го типа, оценить их прогностическую значимость для ранней стратификации групп риска.Материал и методы. В исследование включено 150 диад «мать – доношенный новорожденный», разделенных на три группы по 50 пар в зависимости от наличия и типа нарушения углеводного обмена у матери: 1-я группа – СД 1-го типа; 2-я группа – гестационный СД; группа контроля – без нарушений.Результаты и обсуждения. При анализе течения беременности установлено значимо более тяжелое ее течение для женщин с прегравидарным СД 1-го типа (развитие преэклампсии, стойкая гипергликемия на фоне базисно-болюсной инсулинотерапии) в сравнении с женщинами, развившими гестационный СД и без нарушения углеводного обмена. Новорожденные от матерей с СД 1-го типа имели все признаки диабетической фетопатии (макросомия, различные пороки сердца, неонатальная гипогликемия), которые сопровождались морфометрическими изменениями со стороны гепатобилиарной системы. Персистенция аутоантител к островковым клеткам в сыворотке новорожденных первой исследуемой группы указывает на активность протекающего заболевания матери и является эпигенетически неблагоприятным фактором при формировании иммунной толерантности организма ребенка.Заключение. Прогнозирование склонности новорожденных от матерей с СД 1-го типа к метаболическим нарушениям и оптимизация контроля обмена веществ помогут снизить заболеваемость и улучшить качество жизни этой категории пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To identify factors associated with the development of metabolic disorders in newborns born to mothers with type 1 diabetes mellitus (DM) and to evaluate their prognostic significance for early risk stratification.Materials and methods. The study included 150 “mother – full-term newborn” dyads, divided into three groups of 50 pairs based on the presence and type of maternal carbohydrate metabolism disorder type diabetes: type 1 DM, gestational DM or no disorder.Results. Analysis of the course of pregnancy revealed a significantly more severe course for women with pre-pregnancy type 1 DM (development of preeclampsia, persistent hyperglycemia despite basal-bolus insulin therapy) compared to women who developed gestational DM and those without impaired carbohydrate metabolism. Newborns of mothers with type 1 DM had all signs of diabetic fetopathy (macrosomia, various congenital heart defects, neonatal hypoglycemia), which were accompanied by morphometric changes in the hepatobiliary system. The persistence of autoantibodies against islet cells in the serum of newborns in the first group indicates active maternal disease and is an epigenetically adverse factor in the development of immune tolerance in the child’s body. Conclusion.
